Bliss Sauna Inc. provides a mobile sauna experience that involves high-temperature exposure and potential health risks

Participant Information

Section 1: Acknowledgment of risks

By signing below, I acknowledge and agree to the following:

1. Sauna use with Bliss Sauna Inc. involves inherent risks, including but not
   limited to dehydration, heat-related illnesses, sudden changes in blood
   pressure, slips, burns, environmental risks, and natural events.
2. I assume all known and unknown risks associated with my use of the sauna,
   including those specifically outlined above and any others that may arise.
3. Participation in sauna activities with Bliss Sauna Inc. involve inherent risks that
   may result in severe injury or death. I release, waive, and discharge Bliss Sauna
   Inc., its officers, employees, agents, and representatives from any and all
   liability, claims, or causes of action for injury, death, or damages that may arise
   out of or in connection from my participation in sauna activities, whether
   caused by negligence of Bliss Sauna Inc. or otherwise.
4. I confirm that I am voluntarily participating without coercion and fully

Section 2: Participant responsibilities

By signing below, I confirm my understanding and acceptance of the following:

1. Health and physical condition: I confirm I am physically capable of sauna use
   and accept responsibility for consulting a physician regarding any health
   concerns. Conditions requiring caution include pregnancy,
   cardiovascular/respiratory issues, skin sensitivities, epilepsy, fainting, or heat
   sensitivity.
2. Self-monitoring duty: I agree to monitor my health during sauna use and stop
   if I feel unwell, notifying staff if necessary. I have disclosed all relevant health
   conditions.
3. Behavior and usage: I will follow all instructions and safety guidelines, avoid
   prohibited items (e.g., glass or flammables), and abstain from sauna use under
   the influence of alcohol or drugs.
4. Personal health management: I am solely responsible for managing my
   health before, during, and after sauna use.
5. Responsibility for hydration and self-care: I am responsible for adequate
   hydration and self-care before, during, and after sauna use.
6. Prohibition on tampering with equipment: I agree not to alter, modify, or
   tamper with sauna settings, equipment, or controls.
7. Personal belongings: I understand that personal items brought into the
   sauna are at my own risk, and Bliss Sauna Inc. is not responsible for loss,
   damage, or theft.
8. Cease participation if unfit: I agree to stop sauna use if I feel unwell and will
   follow all safety guidelines, fully assuming risks for any non-compliance.
9. Financial responsibility for damages: I am financially responsible for any
   damage I cause to the sauna or its equipment through negligence, misuse, or
   failure to follow instructions.
10. Responsibility for guests: I accept responsibility for the actions of others I
    invite or include in the sauna, including ensuring they adhere to all safety
    guidelines and proper use of the equipment.

Section 3: Disclaimers and limitations of liability

By signing below, I confirm my understanding and agreement to the following:

1. Disclaimers: Bliss Sauna Inc. makes no guarantees about the health benefits
   or outcomes of sauna use, and it is not a substitute for medical care.
2. Equipment malfunctions: While Bliss Sauna Inc. maintains its equipment,
   unforeseen malfunctions may occur, and Bliss Sauna Inc. is not liable for
   injuries from such issues.
3. No insurance coverage: Bliss Sauna Inc. does not provide insurance coverage
   for injuries or losses. I am responsible for ensuring adequate personal
   insurance coverage.
4. No admission of liability: Assistance provided by Bliss Sauna Inc. staff,
   including in emergencies, does not constitute an admission of liability.
5. Governing law and binding arbitration: This agreement is governed by
   Alberta law, and any disputes will be resolved through binding arbitration in
   Alberta.
6. Prohibition on class actions and legal costs: I agree to bring any claims
   individually and to cover Bliss Sauna Inc.'s legal fees if I initiate a lawsuit
   despite this agreement.
7. Exclusion for other participants’ actions: I understand Bliss Sauna Inc. is not
   liable for risks posed by other participants’ actions in shared spaces.
8. Acknowledgment of mobile environment: I acknowledge that the mobile
   sauna environment varies with location and weather, and I accept
   responsibility for my own safety in these conditions.
9. Privacy and confidentiality: My personal and medical information will be
   used solely for safety purposes and treated with confidentiality by Bliss Sauna
   Inc.
10. Right to refuse service: Bliss Sauna Inc. reserves the right to refuse service if
    my health, behavior, or disclosed conditions are deemed unsafe.
11. Severability clause: If any provision of this form is deemed invalid, the
    remaining provisions remain in effect.
12. No reliance on oral statements: I am not relying on any oral statements by
    Bliss Sauna Inc. staff regarding safety, risks, or benefits.
13. Non-transferable agreement: This agreement applies only to me and is
    non-transferable.

Agreement and final confirmation
By signing below, I confirm that I have read and understood this document in its
entirety. I have had the opportunity to ask questions and voluntarily agree to its
terms.
